Disease – cognitive disorders – institutionalization –agitation – physical and/or chemical restraint … this is the reality of thousands of Portuguese old persons, enhanced by poor caregivers (professionals or relatives) training, boosted by the present restrictive biophysical/chemical medicine, not taking in consideration the mental and social dimension of the human being (WHO recommendation).
Medicine brought years to life, but only through Human Relationship life can be given to the years!
Humanized care is moral and legally mandatory (see "Resumo legal"). But, what are they? What is the state of the art? How can we ensure and certify them?
This is the public utility project that will provide the assertive answers, based on 2 action axes:
1 – individual and collective training for caregivers (professionals and relatives);
2 – Social and health services interdisciplinary diagnosis and action.
Anchored in the results of 5 years of testing and validation of its two main tools, this is a model that will ensure, and evaluate, the "Humanized care" quality in social support institutions.
THE BASE - HUMANITUDE® caregiving methodology, which systematizes and uses the knowledge on the Human Relationship pillars; the Look, the Word, the Touch and Uprightness. Providing consciousness and method to every detail of caregiving, it allows to maintain (or recover) the perception of being a Person, through 150 techniques that professionalizes the humanist philosophy that sustains it.
Guaranteed wellbeing, health benefits of cared ones and their caregivers will be potentiated.
CLINICAL SUPPORT - Comprehensive Geriatric Assessment methodology (CGA), for diagnostic, prescription and interdisciplinary monitoring - physical, mental and social - which substantially reduces the iatrogenic effects and drugs necessity. Added value, that leaves the usual hospital setting and move on to the social support institutions.
5P's medicine: Personalized-Predictive-Preventive-Participative-Proximity
This will be the 1st integrated prophylactic and therapeutic performance system, focused on identifying and maintain cared ones’ potentials, rather than just pathologic symptoms combat.

A partnership with the Santa Casa da Misericordia da Amadora, identified institution as the ideal ecosystem, for this knowledge transfer project, it has 2 goals:
1 – to introduce models’ benefits to its elderly Care homes, Residences, Day Centers, Home Care and Clinic;
2 – to leverage the social business that will allow its national replication (SEE REWARDS).
